You should also know, that if I, as your counselor, become aware of a potential breach of Protected Health Information (PHI) I am legally required to perform a risk assessment, and then mitigate breaches and report them to affected clients, the federal government, and in some cases, the media. A “breach” is defined as the improper “acquisition, access, use or disclosure of protected health information”. Furthermore, there is a presumption of a breach under the above definition unless a risk assessment demonstrates a “low probability” that protected health information has been compromised.

  • As changes occur in federal HIPAA laws, I will make changes to this Privacy Policy and will post changes in my waiting room. I will provide a copy of these changes to you.
